As a Psych Social Rehab Worker, you will provide in-home intervention to children utilizing a cognitive-behavioral, solution focused, social skills training for youths' social, emotional and behavioral well-being by using an accessible service delivery system that is flexible, culturally competent and responsive to the child. Intervention is done primarily in the home, school, and community setting as needed. In addition, you will be responsible for the intervention and treatment process and for assessing problems and providing care to youth. You will monitor individual progress in the attainment of predetermined goals and provide regular reviews and update treatment plans. Additional duties include, but are not limited to the following:
Assesses problems, prepares proper documentation, and treatment plans
Develops and maintains effective working relationships with the youths' treatment team
Maintains appropriate level of direct contact with youth per program and clinical assessment
Coordinates service delivery of discharge planning objectives with community organizations and associated family workers, providing appropriate documentation
Provides consumers, including school personnel and others working with children served by the program, training in the management of difficult behaviors
Develops and maintains effective working relationships with support services for the program, such as doctors and dentists, public welfare personnel, lawyers, therapists, law enforcement staff, school officials, neighbors, religious and youth organization representatives, etc; utilizing referral sources as necessary
Prepares and submits reports and/or recommendations to the direct supervisor concerning case load status, critical incident involvement of family members and staff, budget requests, changes in program plans, and any unusual incidents that occur in the operation of the program
Develops and maintains effective data collection systems, providing required and appropriate data and establishes and implements systems to predict and prevent potential problems
Prepares proper documentation and service plans in a timely manner as defined by the program
Reviews individual staff data to ensure that program guidelines are being followed, and uses the data to make appropriate corrections
Performs other duties as assigned
