To provide professional public health nursing services that promotes maternal, child and family health for individuals and the population in South Hennepin County.

Examples of Duties:
Provides public health nursing services to individuals and families (i.e., prenatal support and education, postpartum and newborn cares, parenting, child health, growth and development, etc.)
Coordinates daily referrals and assignments with other family home visiting nurses
Advises and teaches clients and their families about appropriate health care (physical, functional and psychosocial)
Assesses the health and psychosocial needs of individuals and families and administers plan of care
Provides consultation services and advises local child care centers regarding Rule 3 MN state licensure requirements
Participates in various clinical and community health promotion programs
Makes appropriate referrals to other community resources as needed
Maintains good public relations and communications with community agencies, organizations and health care facilities
Maintains accurate and timely documentation of client services via computer documentation
Receives and answers inquiries relative to public health services availability and/or public health matters in general
Regularly travels to and from clients’ homes, child care sites, etc.
Works effectively across cultures
Report to work in a public health emergency
Performs other duties and responsibilities as assigned

Minimum/Desirable Qualifications:
Minimum Qualifications

Baccalaureate degree in Nursing
Current license as a Registered Nurse in the State of Minnesota
Certification as a Public Health Nurse in the State of Minnesota
Valid driver's license, car insurance and dependable car
One year recent experience in maternal child health (maternity/prenatal, postpartum or pediatric clinical work), public health, or home care nursing
Ability to communicate both orally and in writing with the public and clients in an effective manner
Ability to maintain timely and accurate records, using computerized charting and other software programs
Experience working effectively across cultures
Experience working with families who are at high risk for poor health outcomes (i.e. low socio-economic or mental health issues)
Experience working with Microsoft Office Desirable Qualifications

Additional experience providing home visits to families
Experience providing prenatal, postpartum and newborn care
Experience working with diverse cultures and interpreters
Understanding of effective support for parents with young children
Experience working with interdisciplinary teams
Fluency in Spanish or other non-English language
Experience educating individuals or groups about health related topics
Advanced degree or certification related to maternal child health
