Responsibilities: Integrate, coordinate, and monitor the active treatment program and medical services for each person receiving services. Represents the client to ensure that the individual receives needed services and interventions from providers who deliver quality services. Write training plans, and case conference agreements for persons served in the Supported Living program. Develop, write and implement training plans for persons served regarding medical issues, including BQIS regulations regarding medications and side effects, seizure plans and precautions, dehydration and constipation, nutritional/dietary needs and safety/behavioral needs. Provide individual specific training to new Direct Support Professionals and provide updated trainings as consumers needs/requirements change. Attend and participate in monthly house meetings for persons served and monthly meetings with SSC and Team Lead. Attend and participate in quarterly and annual case conferences and other meetings related to persons served in the Supported Living program. Review monthly data documentation and write monthly progress reports for Supported Living program. Ensure documentation of Supported Living services is completed and accurate, including but not limited to daily notes, MARS, training plans, risk plans, and any other necessary documentation. Coordinate the delivery of approved services between selected service providers. Serve as a liaison between consumers, families, staff, external professionals, and the community. Coordinate and evaluate the instructional activities that are provided to persons served. Complete incident reports, including BDDS, and needed follow-ups. Participate in monthly Case Record Review committee meetings and record review follow-up. Provide direct care as needed and participate in the on call rotation. May perform some staff supervisory functions when on call. Assist with planning and participate in all special Supported Living activities, as needed. Must be able to care for the physical needs of persons served including feeding, hygiene, and toileting. Must be capable of assisting in physically transferring/moving consumers when needed. Physical activities may include but not be limited to: lifting up 25-50 pounds (depending on person served); bending; reaching; stretching; grasping; visual acuity; walking; squatting, guiding; and transporting. Assist with the use of medical adaptive equipment including, but not limited to: Wheelchair, Hoyer Lift, Shower Chair, Blood Sugar Testing Equipment. Assist with the review of billing and time sheets for accuracy and compliance with regulations. Coordinate and transport persons served to medical appointments and/or procedures as needed. Assist Division Manager, Supported Living and Vice President, Social and Community Services as needed.
Bachelor's degree in special education, education, behavioral science, or related field from an accredited college or university preferred.
Previous experience working with persons with disabilities preferred.
Must possess excellent communication skills both written and oral, and strong teamwork skills.
Proficiency in Microsoft Word and Excel.
Must possess or be able to obtain CPR and first aid certification annually and perform as necessary.
Must possess or be able to obtain CPI certification annually and perform as necessary.
Possess and maintain a clear criminal record per Medicaid Waiver regulation requirements.
Maintain a valid Indiana driver's license, insurance on personal vehicle (minimum of PLPD with Medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier.
Must possess and maintain reliable transportation that meets state requirements for transporting persons served.
Must have the ability to successfully work in both independent and group settings. Must have the ability to effectively communicate during a crisis situation.
