Under the supervision of the Licensed Professional the Qualified Professional provides clinical services and support to the Intensive In-Home team and the consumer/family. This position is crucial in the management of therapeutic services and clinical information. The Qualified Professional is responsible for multiple supervisory tasks related to the clinical/therapeutic functions of Intensive In-Home services, including coordinating schedules, maintaining clinical records and the managing multiple service requirements.

Team Management (Administrative)
1. Participate in weekly team meetings to discuss cases
2. Participate in scheduling weekly contacts for team members
3. Monitoring implementation of the PCP and effective clinical intervention

Direct Contact
1. Providing and coordinating the assessment and reassessment of the recipient’s clinical needs
2. Assisting with crisis interventions
3. Implementing a home-based behavioral support plan with the youth and his or her caregivers as indicated in the Person Centered Plan
4. Monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the Person Centered Plan
5. Participating in the person-centered planning process
6. Providing psycho education as indicated in the Person Centered Plan

Care Coordination (Administrative/Direct)
1. Coordinates and oversees the initial and ongoing assessment activities
2. Convening the Child and Family Team for person-centered planning
3. Completing the initial development and ongoing revision of the Person Centered Plan and ensuring its implementation
4. Case management
5. Linkage and referral
6. Communication with collateral (Natural/Community Supports)
7. Development and revision of the PCP and authorization
8. Monthly treatment team meetings
9. Ensuring linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations
10. Providing and coordinating behavioral health services and other interventions for the youth or other family members with other licensed professionals and Child and Family Team members.
11. Consulting with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process
12. Evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person Centered Plan
