JOB DESCRIPTION

Position Summary: The Oncology Nurse Navigator (ONN) provides support for Oncology patients throughout the treatment process to ensure a patient centered care experience. The ONN demonstrates specialized Oncology knowledge, communication skills, and understands the unique aspects of the oncology population. The ONN will facilitate patient education and coordinate access to community resources. The Navigator collaborates with the patient, significant others and all providers in the decision making process. The ONN works collaboratively with the service line and marketing team to promote the program and increase referrals. The ONN will serve as a liaison between the cancer program and referring physicians to streamline the patient pathway. The ONN will measure, assess, and improve processes and satisfaction for the Oncology patient population.

The Oncology Nurse Navigator displays positive support for RMC and affiliated programs and providers. The RN performs duties consistent with his/her scope of practice and functions with the philosophy and overall plan of care delivery at RMC. The following lists (not all inclusive) are expectations of the navigator

Assessment/Evaluation of all new diagnosed pts to identify needs

Coordinate with providers all discharge arrangements including referrals

Scheduling of appointments

Coordination of travel arrangements as needed

Ensure all applicable records are available to providers

Navigating the care from outpatient to inpatient settings

Coordination with Case Mgt around insurance and discharge planning

Patient/Family education regarding the Oncology Program and process

Identify community resources as needed

Perform Nursing functions for the Radiation Oncology Unit

Assessment of needs for social service consult or other disciplines

Make Discharge Calls for follow up to ensure patient satisfaction with process

Meet with community PCPs to introduce the Navigator program

Participate in Multidisciplinary Oncology Rounds

I. Position Requirements:

A. Licensure/Certification/Registration: Current licensure in the State of Colorado as a Registered Nurse; BLS – Healthcare provider.

B. Education: Graduate of an accredited school of nursing –BSN

C. Experience: Two-Five years experience in Oncology Patient care preferred.

D. Special Qualifications: Ability to communicate/advocate effectively with patients, families, physicians, and other health care team members. Ability to manage a team and work with others within a team to ensure quality patient care. Strong critical thinking skills.

II. Degree of supervision required: Involves general guidance and direction by the Patient Care Director, Nurse Manager and Service Line Leadership. Employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures.

III. Populations and Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards. (The skills and knowledge needed to provide such care may be gained through education, training or experience.)
