The Case Manager is available to help patients with social and emotional issues that may affect their medical care and well-being. Responsible for coordination and education of community resources available. Works with patients, families and physicians to facilitate plan of care.
Qualifications
EDUCATION REQUIRED:
Graduate of an accredited school of nursing.
EXPERIENCE REQUIRED:
3 years of discharge planning and UR experience preferred.
SKILLS, KNOWLEDGE, AND ABILITIES:
Demonstrated skills in organization, verbal and written communications, customer service, math, analysis, reading, following instruction, basic computer usage.
LICENSURE/CERTIFICATION:
RN license in the State of Florida. BLS.
