· Performs concurrent review of the clinical documentation in the medical records. This includes a review of all core measure indicators for all potential and actual AMI/HF/PNE/SCIP/VTE and outpatient measure patients .
· Identifies potential core measure patients daily using various sources, i.e., daily census, BNP and Troponin reports, initial admission data, data in Meditech, etc.
· Performs concurrent medical record abstraction using the COMET system per ethical guidelines and maintains a benchmarked timely completion rate.
· Ensures that all deadlines for reporting are met for QNET/COMET, Division Quality and other reporting requirements.
· Supports the BRMC quality framework and acts as a liaison between the quality department and hospital staff.
· Queries the medical staff and other caregivers as necessary via written/verbal communication to obtain accurate and complete physician documentation that supports core measure compliance before and after discharge.
· Serves as resource for core measure compliance for patients, staff, and physicians. Promotes evidence based clinical practice standards with nursing staff through mentorship.
· Continuously assesses processes and provides performance improvement suggestions to peers and hospital leadership.
· Performs medical staff and clinical staff education on evidence based practices surrounding core measures.
· Maintains high level competency of evidence based practice guidelines. Collaborates with the core measure abstractors on changes to abstraction guidelines for current and new measures.
· Periodically teaches educational offerings to clinical staff including physicians.
· Performs a thorough chart review and documents on the core measure checklists and performs follow-up with clinical staff including physicians as necessary.
· Demonstrates creativity and enthusiasm while pursuing the goals of the department and the organization.
· Acts as chairperson for the Core Measure Action Planning Committee Presents core measure and quality data at various hospital committees.
        Minimum Requirements : Education: Registered Nurse, Bachelors required, Masters preferred. Licensure/Certifications: Current Missouri State Nursing License, BLS Experience: A minimum of five years of acute care clinical experience.
        · Knowledge of care delivery documentation systems and related medical record documents.
        · Knowledge of age-specific needs and the elements of disease processes and related procedures.
        · Strong broad-based clinical knowledge and understanding of pathology/physiology.
        · Excellent written and verbal communication skills and critical thinking skills.
        · Ability to work independently in a time-oriented environment.
        · Computer literacy and familiarity with the operation of basic office equipment.
        · Assertive and friendly personality traits to facilitate ongoing physician communication.
        · Working knowledge of core measures preferred
