Independently provides holistic, individualized care using the nursing process for patients routinely seen on the unit. Demonstrates clinical competency, nursing judgment, teaching skills, and a commitment to improving the quality of patient care. Independently performs general and unit specific clinical skills.

Mandatory Requirements

1. Demonstrates the ability to obtain and interpret information in terms of the patient's needs. Possesses knowledge of growth and development, and has an understanding of the range of treatment needs for adult geriatric population.
2. Reviews/revises/records an individualized patient history and assessment utilizing the minimum data set and the Federal Medicare and State Survey Guidelines. Completes adult admission history and daily assessments.
3. Completes minimum data set {MDS} and adjusts the care plan based on the resident assessment protocol
{RAP}. Sees that nursing portion of MDS is completed within two days of patient discharge from unit.
4. Evaluation of the patient's status and response to medical and nursing treatments/interventions is routinely documented.
5. Receives transcribes and carries out physician orders accurately and in a timely manner.
6. Evaluates teaching effectiveness of patient/family and routine documents on Interdisciplinary Patient/Family Education Form.
7. Provides clear, concise and pertinent direction to care team. Determines work assignments for subordinates based on resources, identified patient care needs and nursing staff skill/level.
8. Adheres to compliance rounds of unit.
9. Assists in data collection in cooperation with Quality Improvement and State Survey requirements.
10. Adheres to OSHA guidelines/universal precautions as described in Hospital Policy.
11. Assessment of Patients: Systematically and continually assesses the health and status of the patient
assuring the application of age specific standards.
12. Care of the Patient: In collaboration with the patient/family, with regard to developmental, cultural and
age specific differences, develops and implements patient care plans to include problems, goals,
evaluations and interventions.
13. Education of patient/family: Provides patient/family education with regard to assessing ability to learn,
special learning needs, and evaluating effectiveness of teaching. Documents information in patient/family
education screens on computer.
14. Surveillance, prevention and control of infection: Follows established policies and procedures to identify
and reduce the risks of acquiring or transmitting infection by use of personal protective measures.
15. Management of information: Documents assessments, interventions and evaluations of care objectively
and timely, using the computer technology required by the department.
16. Leadership: Provides clear, concise and pertinent direction to the care team bases on resources, identified
patient care needs and nursing staff skill level, documents critical incidents, uses time and supplies resourcefully, functions as charge nurse as needed.

Qualifications
Minimum Education and Experience:

Licensure/Certification/Registration:

Current Registered Nurse License in the State of Texas, current BLS & ACLS certification.

Education:

High School graduate or equivalent required. Must have graduated from an accredited School of Nursing; BSN desirable.

Experience:

A minimum of 6 months of Telemetry/Critical Care nursing experience is required with 2 yrs or more preferrred; must be able to articulate and demonstrate appropriate nursing theory and practice.

Knowledge/Skills/Abilities:

Demonstrates good organizational skills. Provides safe and appropriate patient care within the Standards of Nursing Practice. Displays appropriate judgment and problem solving skills. Communicates positively and professionally with all internal and external customers. Maintains patient confidentiality.
