Fairview Park Hospital is a leader in healthcare for the City of Dublin, Laurens County, and the surrounding communities. Fairview Park is a state-of-the-art, 190-bed acute care facility providing community healthcare services including a 24-hr Emergency Room, inpatient and outpatient surgery options, and rehabilitation therapy services. In addition, Fairview provides specialty services through facilities such as the Sleep Center at Fairview Park, the Same Day Surgery Center, the Heart Center at Fairview Park, and the Women’s Imaging Center.

Position Summary:

Responsible for the comprehensive assessments/screening of patients for admission to the inpatient Rehabilitation Unit. Promotes effective utilization, monitoring of health care resources and assumes a leadership role within the interdisciplinary healthcare team. Strives to achieve patient and physician
satisfaction as well as optimal, cost effective clinical outcomes. Collaborates with outside agencies, ancillary departments, physicians and nursing department by providing clinical, consultation, education, information and referral support to ensure that quality and cost-effective care is provided. Responsible for marketing of Rehabilitation Unit. Familiar with CMS guidelines and CMS 13.
Population served: Pediatric, Adolescent, Adult and Geriatric.


Previous healthcare experience preferred.


Graduate of an Accredited School of Professional Nursing, Bachelors Degree preferred. RN is preferred

Special Qualifications:

Ability to assess, evaluate, intervene as needed in care delivered to specific patients; Effective communication skills (verbal and written); Basic knowledge of the growth development and range of treatment needed by assigned case group; Interpersonal skills necessary to evaluate patient status, health
care procedures/techniques and to monitor the quality of patient care as well as ability to identify opportunities for improvement and problem resolutions; Fiscal skills to monitor costs and identify potential savings.

Valid driver's license and automobile insurance, background check with DOT.
Familiar with FIM scoring, CMG, UDS and CMS guidelines. Must be flexible and service oriented.
