Provide Leadership and direction to the Case Management Department Develop and implement strategies to facilitate timely and appropriate admissions, treatment and length of stay Assessing and identifying needs of the program, continually improving the processes and functions related to case management using the Intergraded model; monitors and facilities proper utilization of resources Facilitates overall UM and Discharge Planning program within the Revenue Cycle, including Physician Utilization in conjunction with the Physician Advisor and or Chief of Staff Educates and trains clinical leadership and staff as to the utilization management plan and their respective responsibilities related to the plan Measure, analyze, and report outcome data internal/external including length of stay and case mix i ndex and keeps facility within expected ranges Oversees performance improvement initiatives of the department including patient/family involvement in the discharge planning process Coordinate with Pharmacy for high cost drugs, IV/PO conversion and indigent medication replacement and assistant programs Interface with Managed Care Contracting Interface with hospital administration to ensure Case Management goals are consistent with hospital strategic plan Communicate with Corporate Case Management leadership; participate in initiatives Provide leadership and direction to charge/clinical documentation function Oversees training of charging and documentation processes to foster compliance with hospital billing regulations Assist in improving relationships with nursing homes and referral sources in the community Performs other duties as assigned
Is an active member of hospital leadership and understands the importance of leading by example
Active GA RN required, BSN preferred
5 years hospital case management experience required, or 1 year hospital Case Management Director experience required
