The RN Case Manager, in collaboration with the entire Interdisciplinary Team, is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet a patient/family’s end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles, and holistic philosophy, assuring that the changing needs of the patient/family are met while facilitating a pain-free, peaceful, sacred death. Demonstrates personal accountability for own continuous growth and development. Seeks out appropriate opportunities to enhance competency and improve performance.
Graduate of approved School of Nursing. Bachelor’s degree in Nursing or related field preferred. Minimum one year experience in direct patient care; Hospice experience or long term care experience is preferred. Visits private homes and/or facilities. Will work in an unpredictable environment that may include wide variations in cleanliness, temperature, behaviors, smoke, animals, and other potential hazards. May be subjected to possible exposure communicable diseases. Work schedule is variable according to patient and family needs. Works at times in an office setting. Current license to practice as Registered Nurse in Michigan; strongly encourage CPR certification; valid Driver’s License.
