Assesses, coordinates, negotiates, procures, and facilitates the utilization of resources for patients/clients to achieve high quality, cost-effective outcomes. Evaluates and facilitates the appropriate utilization of facilities, services and levels of care. Applies evidence-based findings to develop individualized plans of care based on patient needs and organizational policies, procedures, and protocols aimed at achieving positive patient and family outcomes. Works collaboratively with multi-disciplinary staff, internal and external to the organization. Participates in Quality Enhancement and evaluation processes related to the management of an assigned patient/ client population. The Case Manager is accountable for specific outcomes that are vital to the Case Management process by performing the six essential activities of Case Management of assessment, planning, implementation, coordination, monitoring, and evaluation for optimal patient/client outcomes across the continuum of multiple episodic care coordination activities.
The staff member must demonstrate knowledge of the principles of growth and development across the life span and demonstrate the skills necessary to provide direct and indirect care appropriate to the age of the patients served within the practice. The individual must possess the ability to understand the appropriate information needed to identify each patient's requirement relative to his/her age-specific needs and to provide the care needed as described in the department's policies and procedures.
Arizona State Registered Nursing Licensure or compact state RN licensure.
