Full-time RN Nurse Case Manager for a Chronic Pain Initiative
(in collaboration with Community Care Plan of Eastern Carolina)
Looking for RN with experience working in community health; organized; able to multitask; computer skills a must. Will require travel in coverage area including Halifax, Northampton, Hertford, and Bertie Counties.
The Nurse Case Manager collaborates with the patient and other health care providers to facilitate client access to those health and social services which enhance the patient's health and well-being and his/her ability to adhere to the prescribed medical treatment regimen in the most efficient and cost effective manner possible. The Nurse Case Manager provides services to the patient in a variety of community and clinical settings.
ROLES AND RESPONSIBILITIES:
Accountable for developing, facilitating services, and partnering with community resources to address the needs of an assigned population.
Uses independent, professional judgment to provide case management services to assigned population.
Coordinates and develops educational opportunities related to assigned population for a variety of stakeholders (i.e. the healthcare system, school system, community, employers, health departments' patients and their families).
Provides leadership in population based health improvement.
Maintains and advances knowledge and skills in areas of specialization.
Promotes and participates in quality improvement (QI) activities and clinical research.
Additional expectations of the CPI Nurse Case Manager:
Managing a case load of chronic pain/SA/BH patients that have been predetermined to have high utilization of services. This would involve making regular home and/or professional visits with physicians, specialists, enhanced mental health services providers, social services or other community resources to improve the patients' access to outpatient care.
Attendance expected at formal provider trainings hosted by CCPEC/CCNC. May be asked to assist in providing assistance/education on the management of chronic pain with SA/BH
co-occurring conditions to individual providers or other community service agencies.
Assess and assist in development of resources and relationships in assigned communities to improve the effectiveness of patients' overall plan of care
Participate in local coalitions and collaboratives, as well as promote and educate on the Project Lazarus model, including harm reduction/prevention initiatives with local law enforcement.
BS in Nursing preferred, but will consider associate degree in nursing with related experience with the chronic pain population.
Current unrestricted RN licensure in state of North Carolina or holding a Class P temporary license as a registered nurse in the state of North Carolina.
Knowledge of managed care and population-based disease management concepts and strategies.

A Certified Case Management designation is preferred. Previous experience in clinical resource management activities is preferred.

Basic computer skills, including ability to type; must be able to quickly learn to use eClinicalWorks electronic health record system.
Bilingual (Spanish) is a plus, but not required; applicants who may serve in a translating capacity will be required to take a language assessment
