Responsibilities for this position include:|
Facilitate and promote positive health and wellness outcomes.
Contact members and maintains confidentiality, safety, and advocacy.
Identify the member's current knowledge of condition, psychosocial, physical and environmental needs, and knowledge of healthcare system.
Engage the member in the planning process with direct telephonic communication.
Establish a plan to provide information to the member appropriate for their stage in the decision making process.
Coach members on evidence based treatment options.
Evaluate the member's level of understanding of their condition/treatment options.
Support member in selecting an appropriate physician and/or facility to support a second opinion when appropriate.
Identify referral opportunities to internal/external program(s) and monitor the need for additional care.
Collaborate with the Medical Director on complex cases.
Measuring of outcomes as dictated by the member's response to the information provided.
Documentation of all activities in the system.
Call Center working environment requires sitting for majority of scheduled shift.Work at home opportunity could be consider after 6 months.This is an evening shift position work hours 12:30pm to 9:00pm Monday - Friday
