Responsible for providing patient/family care in consultation with Primary Physician, Team Manager, and Interdisciplinary team in accordance with The Denver Hospice philosophy and delivery of care. Provides appropriate nursing interventions and documentation. Assist patient/families with external resources such as home health, financial concerns, counseling, etc. Educates, and motivates patient and families regarding actions and behaviors towards self-care, wellbeing and self-empowerment. Comfortable working with a variety of different cultures and beliefs. Hours 8-5 Monday-Friday with possible 1 weekend per month and some holidays.
Ø Knowledge of the health care community
Ø Knowledge of nursing assessment for home care
Ø Complete understanding/acceptance of Palliative Care and Hospice philosophy
Ø Knowledge of agency medical record documentation system and process
Ø Able to provide skilled nursing intervention for patient/family as required by state/ federal regulations and certification.
Ø Able to communicate compassionately with team members, patient/family and community.
Ø Able to educate patient/family and community about hospice, palliative care and The Denver Hospice philosophy and nursing care.
Ø Able to demonstrate flexibility, dependability, professionalism, and maturity
Ø Able to cope with crises
Ø Able to function effectively with assigned technology
Ø Current valid Colorado RN license
Ø One year experience in hospice/palliative care, med/surg, public health, oncology, acute care, or home health
Ø Baccalaureate degree in nursing
Ø Three or more years experience as a nurse
Ø Hospice/ Palliative care experience
