- Bachelor's Degree Required
EDUCATION: Graduate of an accredited school of nursing with Bachelor's Degree in Nursing or related healthcare field

EXPERIENCE: 3 years clinical experience in case management. 5 years leadership experience with focus on case and disease management. (i.e. utilization/quality/care/case managment and/or manager of physician practice)

LICENSE/CERTIFICATIONS: Possession of current Texas State License for Registered Nurse

PHYSICAL REQUIREMENTS: Must have eye/hand/foot coordination, finger/fine dexterity and be able to handle/grip/squeeze and perform repetitive acticity. Must be capable of transferring-vertical/horizontal, lifting/carrying 20 pounds and be able to push/pull, climb, stoop/bend, kneel, walk, sit, stand, hold and reach.

SUMMARY: Provides leadership for the system development and execution of strategies to improve the acute care patient's transition to the next level of care. Organizes cross functional teams to address the transitions between the hospital and Home Care, Long Term Acute Care and Skilled Nursing Facilities. Develops strong relations with preferred post-acute partners to creat clinical pathways and goals for management of chronic illnesses to decrease 30-day readmission rates. Accountable for executing, monitoring and evaluating system and site initiatives. Develops strong relations with hospital senior management, key medical staff and nursing to facilitate effective and efficient transitions from the hospital. Accountable for establishing the discharge operational integration of Accountable Care Organization (ACO) patient with the hospital services and the ACO.
