Position Summary: Reporting to the County EMS Medical Director, County EMS Administrative Manager, and the Divisional General Manager, the RAP Program Manager is responsible for the structural and operational activity of the CIty's EMS Resource Access Program (RAP). The RAP Manager implements all dimensions of the program and works on advancing its operational capacity.
Essential Functions: Program Development: • Initiate and establish formal relationships with external organizations • Develop and implement strategies of intervention to effectively guide patients into appropriate methods of care • Collaborate with City Attorneys to ensure licitness of program practices • Participate in preparation and presentation of proposals for possible program funding. Technology Development: • Conceptualize electronic monitoring methods and collaborate with current technology providers. • Guide the development of EMS specific electronic case management tools • Participate in community health and social information technology development. Clinical Activities: • Monitor 9-1-1 activity of frequent EMS users • Investigate background and circumstances of frequent EMS users • Consult with the EMS Medical director to develop individual case plans • Contact or visit patients wiht difficult social or medical circumstances • Respond to scene of emergency calls, where appropriate and productive • Refer patients to voluntary or mandated treatment plans • Participate as a member of the City's Homeless Outreach Team • Serve as point of contact for outside organizations involved in clinical care or outreach • Strategic management of patient population • Effective resource management. time Management: • Ability to autonomously manage daily schedule • Flexibility to respond after hours to patient and program needs. Additional Duties: • Various duties, tasks and special projects as asked or assigned.
Qualifications: • BA/BS preferred - Licensed Paramedic (California with 5 or more years of experience) - All necessary certifications to be a licensed Paramedic - City/County Paramedic License (in which program is operated) - High computer literacy - Software design and development experience - Current state Driver's License in good standing - Satisfactory driving record in accordance with the Company's Driving Policy - Excellent communication and customer service skills - Ability to work cooperatively with other employees and medical agencies to provide the highest level of patient care - Ability to read and interpret all required documents and write required reports and correspondence - Other qualifications/certifications specific to the local operation and/or contract requirements - Satisfactorily meet Company physical assessments/tests.
