To assist patients and their families in the home care setting with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Interprets and facilitates use of community and financial resources available to the patient and their family.
Demonstrated knowledge & skills necessary to provide care primarily to the geriatric population & to a lesser degree pediatric & adult population. Demonstrated knowledge of the principles of growth & development over life span. Assess data reflecting client’s status & interpret information needed to identify client’s requirements relative to age-specific needs. Requires excellent communication & human relation skills. Must demonstrate organization, reliability & ability to work with little direct supervision.
Capable of stooping, bending, stretching, & lifting. Tolerate variable amount of standing, walking & sedentary activity. Possess manual dexterity to handle & manipulate equipment & appliances. Rotate different hours, shifts, and/or assigned areas, as well as called in during emergencies to provide adequate coverage. Ability to control emotions & maintain composure under stress, using tact & good judgment & able to adjust to various personalities & situations.
Master’s Degree in Social Worker. Minimum of 1 year healthcare experience. Home Care and Hospice experience preferred. Must be licensed by the State of Illinois as a Licensed Social Worker (LSW) or a Licensed Clinical Social Worker (LCSW), & have CPR Certification.
