Directs & facilitates functions & activities of the department in absence of manager, including receives referrals, completes intake assessment process, p erforms home visits as needed, c oordinates care plan development, interdisciplinary/interagency communication, & activities. Intra-agency coordination provided as needed, coordinates day to day staffing & resource allocation, participates in quality improvement, r eceives & processes physician orders & crisis calls from patients, families and Home Health/Hospice staff when primary nurses are not available, m aintains skills, and other duties as assigned and needed by the manager . Assists manager with development & evaluation of department personnel & operational aspects of department. Promotes nursing professionalism within the hospital. Have minimal unplanned absences from work, arrives at scheduled times, does not abuse sick time or personal days off. Demonstrates knowledge of infection control practices, safe equipment use, safety procedures & appropriate actions in hospital, accountability following hospital policies, guidelines and references.
Must be capable of stooping, bending, stretching & lifting, tolerate variable amounts of standing, walking and/or sedentary activity during shift, p ossess manual dexterity to handle and manipulate equipment and appliances, rotate different hours, shifts, and/or assigned areas, as well as be called in during emergencies, ability to control emotions & maintain composure under stress, using tact & good judgment, a djust to various personalities & situations, work effectively in an environment which tends to be tension provoking, c ommunicate effectively with patients, doctors, co-workers, other departments as well as external entities, work well under supervision & be able to take constructive criticism, well groomed & poised at all times.
Associate Degree in nursing is required (Bachelor’s preferred). Minimum of 2 years of home health/hospice nursing and a minimum of 4 years of nursing exp. Must be a Registered Nurse and possess a current IL license & CPR certification.
