Uses a strengths-based, Solution-Focused approach to contribute to optimal health outcomes of patients by assessing and intervening on psychosocial/economic problems. Interventions may include diagnosis/condition related supportive care, financial and legal assistance, and community resource referrals. Provides social work services to patients and families in a strength-based orientation, with respect for patient confidentiality, individual needs, and patient self-determination; conducted in a manner which promotes and respects the worth and dignity of the patient. Provides clinical assessment, which may include Diagnostic and Statistical Manual of Mental Disorders-IV (DSM-IV and provide recommendations to physicians. Ensures that multidisciplinary care planning is patient centered; considers all aspects of the patient/family's; personal, psychosocial, economic and cultural needs. Provides leadership for clinical care plan development and oversight for specific behavioral patient populations identified through case finding and referral criteria. Coordinates resources by making clinically appropriate and cost effective referral recommendations and organizes treatment services with various community resources. Provides crisis intervention services to all patients deemed appropriate by the ED physician. Provides evaluation for chemical dependency and psychiatric care, providing the recommendation for appropriate level of care. Serves as a team member and support to Emergency Department in all care needs, as appropriate. Serves as a liaison and advocate for collaborative practice between Behavioral Health, Case Management and the ED. Serves as the team leader in recognizing and diffusing violent behavior in the Emergency Department. Certified in CPI: Non-Violent Crisis Intervention.

Qualifications

Masters Degree in Social Work. Licensure (or work toward completion of licensure) preferred.

Working knowledge of interviewing techniques and the ability to assess patient/client needs and establish a plan of action.

Ability to assume responsibility for personal and professional growth.

Knowledge of community resources, their accessibility, function and effectiveness.

Knowledge of NASW Code of Ethics-accepted principles of social case work, group work, and community organization.

Ability to work cooperatively with multidisciplinary team.

Ability to communicate effectively with others.

Ability to be flexible and creative in identifying resources to meet patient and organizational need.

Belief in maintaining respect for individuality, rights and dignity.

Able to meet the physical demands as described in the ADA forms on file in the Human Resources Department.
