Job Summary

Job Purpose: Provide direct and indirect social work services (i.e. assessment, provisional diagnosis, supportive guidance, treatment planning, intervention, referral, and follow-up) to Golden Cross Academic Clinic patients.

Supports the mission, vision, values and strategic goals of Methodist Health System.


Masters Degree in Social Work

Licenses and/or Certifications (Required and Preferred)

LMSW required

Work Experience: No work experience required

Assesses the effect(s) of patient’s psychosocial situation on medical and mental health needs via face-to-face or telephonic interviews through the utilization of the Center of Epidemiological Studies Depression Scale (CES-D), NIMH: Once depression has been diagnosed, crisis counseling and/or Solution-Focused Therapy is performed as deemed necessary in order to stabilize patient’s emotional state until they are able to meet with referral source.

Attends continuing education courses to stay abreast of current social work best practices. Researches local, regional, state, and federal agencies/organizations which provide assistance to patient population.

Completes required documentation including psychosocial assessments, referrals to community agencies, and applications to pharmaceutical companies soliciting assistance through patient assistance programs.

Oversees work of volunteers

Performs tasks necessary to ensure the smooth operation of MedAssist program at Golden Cross Clinic. These tasks include but are not limited to the following: database management, preparing of monthly/quarterly productivity reports, providing educational materials to patients and members of the community, collaborating with community agencies to provide comprehensive medical and psychosocial care.

Serves as a liaison between patient and clinical team including: physicians, nurses, dietician, and health educator to assist with continuum of care.
