Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus by interviewing the patient and his/her family members.
  • Participate in the development and implementation of treatment plans, ensuring that concise, realistic care plans are maintained and carried out.
  • Provide individual, group and family solutions-focused interventions using appropriate therapeutic techniques and models. Facilitate process-oriented and didactic groups.
  • Document treatment and case management provided to patient in medical record legibly and in a timely manner.
  • Develop and coordinate an individualized discharge plan for the patient by utilizing information obtained from the patient, treatment team and medical record to determine discharge planning needs. Identify available appropriate family or community resources such as group homes, halfway houses or treatment providers.
  • Attend and participate in regular interdisciplinary treatment team meetings to provide Social Services perspective to total case management of the patient. Provide communication regarding patient’s case, resources and progress towards treatment plan and discharge planning goals. Make treatment recommendations and actively coordinate each case with all members of the treatment team.
  • Report incidents or suspected incidences of sexual/physical abuse, neglect or exploitation of a minor, elder or disabled person to appropriate state or community protective agencies. Represent the facility in legal proceedings as required by law.
  • Actively address safety issues as part of discharge planning process.
  • Assess patient’s stage and extent of chemical use when appropriate
  • Communicate with referral sources and other appropriate parties regarding patient’s progress.
  • Provide thorough communication to Social Services staff regarding status of assigned tasks. Provide clinical updates regarding patients to facilitate effective continuity of care.
  • Participate in department and other program or clinical and administrative meetings. Support maintenance and development of clinical program services.
  • Adhere to all safety policies and safe work practices.
  • Adhere to all hospital policies and procedures
  • Perform other duties as assigned.

  • Requirements

    Individual must hold a Master’s Degree in Social Work, Counseling or related field. Candidate must be licensed or license-eligible in Georgia. Individual is required to have experience working with adults in an inpatient psychiatric setting. Case Management experience preferred. Candidate must be able to work every Saturday and Sunday, 10 hour shifts.

    Bilingual Spanish a plus
