The Social Worker-Adult Mental Health assists adults with Serious and Persistent Mental Illness (SPMI) diagnosis in achieving and maintaining mental health stability and maximum independent living within the community. Helps adults with SPMI gain access to needed medical, social, educational, vocational, and other necessary services as they relate to his/her individual mental health needs.
Examples of Duties:
- Facilitate Diagnostic Assessments when appropriate. · Complete Functional Assessments. · Complete, implement and update Individual Community Support Plans. · Make referrals to other needed services. · Monitor all client placements and/or services. · Attend all reviews, staffing and discharge planning. · Provide ongoing monitoring of case management services. · Complete all required documentation.
The job requires a Bachelor’s degree in Social Work or related human services field and two years' supervised experience working with adults in the mental health field. Valid Driver’s license required. Must be eligible for employment through the Minnesota Merit System.
PREFFERED QUALIFICATIONS: Master’s degree in Social Work and experience working in county social services.
HOURS : Normal business hours are 8:00 am - 4:30 pm, Monday - Friday, with a one hour lunch break, making a 37.5 hour work week. Full-time.
