Summary of Job Duties
Under the supervision of the Senior Social Worker, the Social Worker renders clinical psychosocial services to the patients and their families of all ages (neonate to geriatric). The Social Worker assesses patient’s psychosocial needs and makes referrals to appropriate facilities or community agencies; and provides crisis intervention and bereavement counseling. The Social Worker is instrumental in the transition of the patient from acute care to transitional care status or from the transitional care to continued extended skilled care. The Social Worker collaborates with the Case Managers and other allied personnel in diagnosis and treatment planning, helping patient/family resolve home, discharge planning, and community problems that compromise effective health care.
Qualifications & Requirements
Ability to adapt to a rapid change in both internal and external environments
Ability to effectively communicate verbally and in written format
Ability to organize and prioritize workload in order to meet deadlines
Demonstrates ability to effectively utilize a computer and Microsoft Office
Masters Degree in Social Work (MSW) from an accredited school, licensed Clinical Social Worker in California preferred
Post-MSW experience in a social agency, hospital, or other organization dealing with health and/or welfare
Willingly performs other duties as assigned
Works well with others in a team-oriented environment, including support of other co-workers’ job duties in a manner that further advances the goals and objectives of the whole organization
