The Social Worker is responsible for providing direct psychosocial counseling and support to patients and families, keeping within the spiritual/religious/cultural frame of reference for each individual. The Social Worker, through a collaborative process, is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet a patient/family’s end-of-life psychosocial needs. An outcome-based plan of care is created and continually adjusted through utilization of the relationship centered care principles and holistic philosophy, assuring that the changing needs of the patient and family are met while facilitating a pain-free, peaceful, sacred death.


Bachelor's Degree in Social Work required; Master’s Degree in Social Work preferred. Must be licensed in the State of Michigan. Demonstrated ability in individual/family/group counseling, including a minimum of one year of clinical work experience, preferably in the health care field required. Must have the understanding and commitment to end of life care. Valid Driver’s License required.
