DUTIES
Provide quality, cost-effective nursing care:
Complete physical assessment on each patient.
Assess caregiver/family needs/goals upon admission.
Provide patient/family with information about hospice and philosophy of care related to their goals.
Develop, implement and evaluate plan of care for each patient.
Complete Nursing CNA Plan of Care; supervise CNA care.
Provide direction to Administrative Assistant and volunteers as needed.
Medication management through safe and cost effective recommendations and usage; Pyxis use and medication reconciliation.
Complete documentation in a timely manner as required.
Collaboration with team:
Participate actively in interdisciplinary team meetings (IDT) when available.
Contribute to the team's understanding of patient/family issues demonstrated through daily report and communication with team members when not at IDT.
Obtain orders and communicate with hospice medical director.
Assist with patient transitions to other facilities and other levels of care.
Coordinate and complete patient admissions to the unit with Access Center to ensure safe and efficient transfer.
Provide patient/family support throughout their stay and at the time of patient death. Provide patient/family education about medications, fall risk, oxygen, disease process, dying process and at the time of death.
Assist with education/orientation of new personnel, students and physicians.
Maintain communication
With attending physician, medical director, clinical manager and outside referral sources regarding patient care as appropriate.
Provide clinical updates and death notification to physicians, insurance case managers and other outside referral sources as appropriate.
Participate in performance improvement through:
The development/tracking of quality indicators.
Address patient/family complaints, attempt to resolve and communicate such concerns to manager for follow up.
Report incidents.
Complete documentation on all of the above as required.
Adheres to the practice of confidentiality regarding patients, families, staff and the organization
Understands that he/she may be exposed to Protected Health Information (PHI), and ensures that he/she treats PHI according to human resources' policies and procedures
Night Sift Esential Functions:
Chart reviews.
Review MARS/M.D. orders
Audit charts for completeness of forms.
Complete Allscripts nightly.
Complete Pyxis count/POM narcotics.
Check medication refrigerator temperatures nightly.
Update nursing care work sheet and CNA worksheet every night.
Complete tasks usually completed by AA as possible related to patient care.
The percentage of some essential functions will change for IPU nurses from shift to shift.
Charge Nurse Responsibilities:
Level of involvement with floor activities varies from nurse to nurse. Some focus primarily on patient related activities while others also oversee issues related to floor management. (Checking if refrigerators are clean, food disposed, etc. and directing CNAs to attend to these things.)
Often the go to person for clinical nursing issues. A trouble shooter/go to person for clinical issues.
Addresses staffing issues. Calls staff off, calls staff to work additional hours to cover a call in, calls for agency staffing.
Coordinates workload of nurses and CNAs per shift.
Orders routine medical supplies.
Coordinates admissions.
Receives calls about potential new admissions to the unit. (Calls coming from field staff, primary/hospice physicians and Access Center.)
Reviews with callers insurance, DNR and LCD status; primary and hospice physician notification; encourages hospice and primary physician contact as necessary.
Puts names on board.
Triages admissions with IPU Medical Director of the week.
Trouble shoots medical supply and equipment needs for new patients.
Take general and family complaint calls asking for "charge nurse".
Orders rescue packs. (Done on pm shift)
Coordinates/monitors housekeeping and rooms cleaning issues related to patient care.
The following responsibilities are undertaken by a select few:
Review of billing from hospital.
Reviewing and submitting updates, changes, etc. to HR every pay period.
Scheduler:
Completes schedule for nurses and CNAs
(This is coordinated among 41 FTEs and multiple "00" staff and completed by reviewing their approved time off requests, attempting to honor other requests and filling in gaps with "00" staff and if need be agency nurses. The manager is made aware of scheduling difficulties and offers input as needed.)
QUALIFICATIONS
Education
ASN required, BSN preferred
Current Illinois RN license.
Experience
One (1) year nursing experience, Prefer two (2) or more years hospice or med surg nursing.
CPR certification preferred.
Demonstrated ability in nursing practice.
