St. Rose Dominican Hospitals Case Management Department provides Case Management, Utilization Review and Social services to an all encompassing patient population, inclusive of a full service line. The goal of the continuity of care function is to define, shape, and sequence the patient’s clinical care and activities along the hospital admission and care continuum through to discharge. The Case Manager bases care recommendations on information gathered from many sources and team members, about patients’ needs and insures the patients are placed in the appropriate level of care through the application of Interqual criteria.
The Utilization Review Nurse is responsible for coordinating healthcare services across the continuum of care by monitoring, evaluating, and modifying the treatment plan to achieve optimal patient outcomes under the direction of the Manager and Director of Case Management. The registered nurse employs critical thinking to integrate objective data with knowledge gained from an assessment of the subjective experiences of patients. Registered nurses use this critical thinking process to apply the best available evidence and research data to the processes of diagnosis and treatment. Nurses continually evaluate quality and effectiveness of nursing practice and seek to optimize outcomes. Medical/clinical knowledge, technical and clinical skills, clinical judgment, interpersonal skills and communication as indicated in the current Comprehensive Accreditation Manual for Hospitals are essential components of nursing practice. All registered nurses practice within the scopes and standards of practice set forth by the American Nurses Association. Additional Professional Organizations may be referenced when appropriate to guide practice and standards within the work place.
This position is represented by CNA and is covered by the terms and conditions of the applicable collective bargaining agreement.
Require Patient Population tab: No
Minimum - Two (2) years acute hospital clinical experience as a registered nurse. One year of Health Care related Case Management experience.
Preferred - Two Years (2) of current hospital Case Management experience or Case Management experience in related field of health care.
Minimum - Graduate of an accredited school of nursing.
Preferred - Bachelor of Science in Nursing.
Demonstrate the ability to work independently and with a team; read and analyze clinical data; be aware of available community resources and regulatory requirements pertinent to case management; uphold patient and family confidentiality; implement quality improvement initiatives; solve problems effectively including conflict management; communicate effectively - both verbal and written.
Required: RN = Current license as a Registered Nurse in the State of Nevada.
CT CPRBLS = Basic Life Support CPR.
Preferred: Case Management Certification.
