Provide care and support for youth primarily in Essex County and the North Country in New York . Services needed include skill building, family and caregiver support, planned and crisis respite, community advocacy, pre-vocational/supported employment, crisis avoidance and response, and in-home supports.
Valid NYS driver's license, vehicle insurance coverage, ability to travel independently within the community.
Schedule will require some evenings and weekends.
Specific duties include the following:
Skill Building - Support, coach, and guide children and their caregivers for successful functioning in the home and community.
Family / Caregiver Support - Work with individuals and their caregivers one-to-one or in small groups, in order to enhance the abilities of the children to live within a family setting and to help the families provide care in the home and community.
Planned Respite - Provide planned short term relief on a one-to-one basis, care for children allowing family and caregivers the ability to support the child's needs.
Crisis Respite - Provide immediate short-term relief, support crisis resolution and reintegration of individuals into family / caregiver structure.
Crisis Avoidance/Response/In-Home Supports - Train individuals and their caregivers to identify triggers, behavior patterns and safe alternatives to avoid crises. Address specific situations that jeopardize participant success in school, community and home. Provide on-call assistance for one to one on-site de-escalation, resolution, and stabilization planning.
Pre-vocational/Supported Employment - (Job coach) to teach adolescents ages 14 and up the skills to search for and retain employment. Work 1:1 or in small group settings after school, evenings, weekends.
Special Needs Community Advocacy - Liaison with community and educational systems regarding the child's strengths, disabilities, and needs related to unmet health care issues. Train the participant and or caregivers in managing transitions within and between community systems.
About Bridges to Health:
B2H provides non-Medicaid Services designed to improve the safety, health, and well-being of a child in foster care, prevent institutionalization, and follow the eligible child through foster care placement, return home, or adoption. B2H also serves foster, birth, and adoptive families.
Bachelors degree and 2 years related experience required
Experience must be working with children, families, caregivers in a community-based foster care setting
Valid NYS driver's license and vehicle insurance coverage
Able to travel independently within the community
Must be willing to work in participants' homes
