Don't ask for feedback and input on programs then penalize those who are brave enough to provide it. Look at reasons for high turnover in parts of the company. Shake up the old guard of managers who have been their for 15 or 20 years and all think alike.
Don't pretend to want input from the field then penalize those who are brave enough to speak up. Rotate out the old entitled management that doesn't follow up with employees and have no new ideas as their experience is limited to CARFAX for the last 15 or 20 years.
