Our Complexivist® providers - physicians, nurse practitioners and physician assistants - are supported by a fully-employed multidisciplinary team composed of nurse care managers, social workers, behavioral health specialists, pharmacists and dietitians. Our providers are committed to administering comprehensive and compassionate care to our patients, wherever they reside and whenever they need it.
Our mobile team of providers extends office-based care into the home, relaying valuable clinical information back to the physician community and ensuring continuity of care across all clinical settings. We do not replace the primary care provider, but support and augment their work in the home.
