Once a year

As far as I recall it was annually.

Rarely. Reviews are once per year. If you're in support staff, you are unlikely to receive a raise for two to three years and it will be slightly less than inflation. If you're in one of the professional roles, any bonus you receive is strictly contingent on charging clients as much as possible and steering clear of the in-house cliques.
