Been working there about 4 months. In the kitchen we have a few people with bright pink/and bleached white hair but I don't think servers are allowed to (at least I haven't seen any). I myself as well as a few other servers have visible tattoos so I think that policy's been changed since some of these answers
No weird hair colors that arent natural colors. Any shirts worn under uniform have to be black as well.
We have people with vibrant hair color (including our culinary manager) visible tattoos and facial piercing the policy changed on hair color last fall...I for one have a bit of pink in my hair other than that the certified trainers comments below are also correct

I'm a Service Pro and Server Trainer: hair must be pulled back (girls: half up half down is okay as long as it's pulled back) no visible tattoos, no facial piercings (gauges are allowed but must be solid-no opening in the gauge), earrings must be no larger than the size of a quarter, necklaces are typically up to the discretion of the stores manager but typically only small simple necklaces are permitted. Employees MUST wear nonslip shoes, no exception.
Uniforms: black button up collar shirt (undershirt must also be black if showing). Black belts and black shirts are required as well as long black dress pants (women) or black slacks (men). No yoga pants, no black pants or black large pockets on the side. Shirts must also have black buttons (seems obvious but sometimes it's hard to find). Headbands are allowed but color and style are also typically up to the discretion of the stores manager. Sleeves can be rolled up but only twice.
No visible tattoo or facial piercings allowed. Black pants black button down dress shirt at collar to. Non slip shoes a must checked regularly. Anything else is ok.
