They don't. Literally. They don't give them and you get a measly 2.5 percent performance bonus (which is NOT guaranteed) once a year but obviously if you leave prior to that, you will not have gotten any monetary incentive. In today's low unemployment, they expect people to keep working for them with no pay increase. The cost of living goes up so should your pay rate. Garbage company.
Once a year. Decisions are made sometime in Q1 and effective April retro back to 1/1 meaning you get a lump sum in April and then increased salary moving forward. If you aren't with the org in April you basically don't get a raise and BTW you must have worked the ENTIRE calendar year to be eligible for a raise (they never tell you that initially but it's in the handbook and that's how it works).
Rare. More common is additional work demands with no additional pay.
