Look at replacing middle management (regionals and especially regional vice presidents) Many are 'yes men' that lack leadership in changing times like we've had for the past year and it shows.

Retire upper management, and fire HR department
Care about the therapists who work their hind ends off. Believe your therapists when they are dealing with toxic DORs or regional managers. Hire an HR department that actually does their job. Allow therapIsts to use their PTO according to their facility department scheduling and not allow regionals to dictate their lives and get back to you when they feel like it. You are unable to make any travel plans due to slow approval of PTO request.
If you treat your people right and you treat yor patients with kindness and caring you will always win. Life in general must be a win, win situation or everyone loses
I would hire a transport tech and part time DOR assistant to do paperwork and filing.
Ensure greater job security for it's employees.
Give people raises!! We work darn hard to make you money. Bet you didn't cut any corporate salaries or positions. Respect the hard work we had to do to obtain our licenses and treat us like the professionals that we are!
I think that it would be good if the PTO could be cashed out when departing from the company.
