Once per year however percentages of raises have gone down from 3-5% to 1-2% no matter how good your review is.

I was use to getting at least a 3-5% raise every year before I became an Administrative Support Assistant. I always had an excellent evaluation. Once I became an Administrative Assistant, I never got a good evaluation or a decent raise.
