The fragile egos of upper management who used the "leave alone, but zap" style of leadership. Leaders didn't know what they wanted, nor how to communicate what they wanted, but 'zapped' subordinates who failed to deliver what they wanted. They was also A LOT of favoritism and cronyism. People were advanced into positions despite the fact they lacked the basic qualifications needed to hold that position.
