Location: On- site at SALTA in United Wholesale Mortgage
Terms: Full-time salaried position – salary based on size of member panel and merit – 4-day work week in office with 1 day out of office but accessible to your panel members.
Accountability Chart: Reports to Office Manager
Qualifications: The APP shall be a graduate of a certified program, shall have passed the national certifying exam, and shall have a current license from the state of Michigan.
JOB FUNCTIONS: Maintaining clinical services standards is one way to ensure our organization is achieving continued service excellence. The APP will Interface on a day-to day basis, with clinical and administrative staff to treat and evaluate members under the supervision of a licensed physician.
- Obtain and record history and physical examinations. Histories and physicals must be reviewed and signed by the supervising physician.
- Treat and evaluate members with and without the physician present including but not limited to acute care, chronic care, procedures, in-office and virtual consultations.
- Reduced total member panel of 800, with the expectation of seeing 9-14 members a day.
- Timely member communication within 48 hours of receiving any laboratory or imaging results.
- Evaluate and integrate new technology to empower the Advanced Primary Care model.
- Establish the health coach infrastructure to engage in population health management activities inclusive of care coordination, closing gaps in care,claims analytics, population risk stratification, and managing/analyzing social determinants of care, integration of behavioral health and weight management strategies.
- Work with administrative leadership on a process of continual quality improvement and lean process management.
- Responsible for several activities related to the delivery of Advanced Primary Care and clinical services such as clinical systems improvement, quality assurance, and medical protocol development.
- Responsible for development and innovation of the Advanced Primary Care model to empower the delivery of the quadruple aim.
SALARY RANGE: $120,000 - $150,000
