JOB DESCRIPTION: Full Time – Aftercare Coordinator
SUPERVISOR: Client Services Manager
HOURS: 40 hours or more per week
CLASSIFICATION: Exempt, At-Will Position. *Grant funded position

Position Summary:
The Aftercare Case Coordinator manages the Aftercare Program. The position is a full-time, salaried position with benefits. The ACC position provides case management to clients who exit the Unaccompanied Youth Program and are eligible for Aftercare Services. The average caseload consists of 15-20 clients. The ACC is responsible for collaborating with the UY Case Coordinator to assist the client during their transition period. The transition period consists of assisting the eligible client in identifying appropriate housing, coordinating utilities, identifying resources, and meeting with each client on a regular basis to provide a continuum of care for up to 12 months. The ACC will provide family follow-up and support for youth reunifying through the Basic Center Program. The ACC will also build partnerships with landlords in the community to secure affordable housing for clients. Personal car will be used and mileage will be reimbursed. ACC must maintain required documentation and make use of HMIS and Apricot databases for client services.

In addition to the Aftercare Program, the ACC will manage the agency’s National Safe Place component. National Safe Place requires that the agency representative (ACC) be on call 24/7 and ensure that they are able to meet with a youth in distress at a partner agency within 40 minutes upon receiving a phone call. (Note: the average number of calls per year is four.) The ACC will be responsible for completing brief, monthly online reports to National Safe Place.

The ACC will attend and participate in relative meetings at community agencies and partners. The ACC will coordinate with the Director of Programs, Special Programs Coordinator, Case Coordinator, and Executive Director to perform other duties as necessary.

Primary Duties and Responsibilities of this Position
The Aftercare Coordinator will implement the Safe Place Program and perform case management for aftercare to support youth 12 months after they exit Palmetto Place. They will be responsible for the following:

Aftercare:

  • Assist youth/young adults 12 months after they exit Palmetto Place Programming.
  • Support clients with skill building and independent living skills through workshops
  • Support case coordinators in client preparation through all programs
  • Provide comprehensive advising and wrap around support to aftercare clients.
  • Implement various retention efforts through monthly independent living workshops.
  • Work in partnership with Community Projects Coordinator to build relationships with landlords
  • Manage the Aftercare Social Media pages
  • Maintain lease and compliance files in an orderly, up-to-date manner. Provide ongoing, active outreach and creative engagement to youth.
  • Link clients to community resources.
  • Support tenants with conflict resolution among tenants and neighbors
  • Meet documentation requirements as dictated by program need

Safe Place:

  • Develop and implement, and evaluate annual Safe Place goals and plans.
  • Assist Community Outreach Coordinator in recruiting businesses and public locations to serve as Safe Place sites.
  • Maintain community relations with key school personnel, business contacts, and community organizations.
  • Conduct school and other presentations to youth for the purpose of explaining the services offered by the agency
  • Maintain Safe Place sites.

Types of Sites Local networks of Safe Place sites should consist of both for-profit and not-for-profit locations.

Businesses: fast food chains, convenience stores, grocery stores, malls, skating rinks, movie theaters, etc. Community services: transit systems, fire stations, libraries, community centers, YMCAs, Boys and Girls clubs, schools, healthcare clinics, youth service organizations, etc.

Supplemental Job Duties:

  • Attend staff meetings, retreats and professional training sessions
  • Participate in on-call Safe Place rotation and occasional evening and weekend work as necessary
  • Other duties as assigned.

Schedule:
The schedule is flexible to meet the needs of clients and the community. Must fulfill a 40 hour work week with at least two day working from a Palmetto Place specific location.

Qualifications:

  • Educational requirements: Bachelor’s Degree in Social Work or related plus 2 years of experience working with at-risk population ages 18-24.
  • Proficiency in Microsoft Office, including Excel, Word and Outlook.
  • Desire and ability to work with and communicate effectively with unaccompanied youth ages 18-24.
  • Client focused, customer service disposition.
  • Ability to be a staff team player working together to achieve the goals of the agency.
  • Ability to serve as a role model for youth.
  • Must be able to work independently with minimal supervision.
  • Must exemplify the ability to advocate internally and externally for clients, obtain and secure resources, and take initiative to grow the program.

Conditions of Employment:

  • Satisfactory reference and background investigation checks (including DSS Central Registry, Fingerprint, SLED, and National and SC Sex Offender checks)
  • Completion of pre-employment drug screening, physical and required forms
  • Completion of Trauma-Informed Care training within first month of employment
  • Possess a clean and valid South Carolina Driver’s License
