JOB SUMMARY:

At RIPCPC, the Integrated Behavioral Health (IBH) Clinician is empowered to utilize his/her strengths in clinical knowledge across the age range while providing acute behavioral health support and treatment planning to patients.
You will be trusted to apply your skills and experience while also being able to lean on the support of fellow clinicians, Community Health Workers, and creating synergistic care within an interdisciplinary care team of Nurse Care Managers, Pharmacists, Quality and Data professionals, and administrative support.

Primary Responsibilities:
You will bepart of Rhode Island Primary Care’s (RIPCPC) ambulatory care team. The Clinician’s role is to support patients in managing their acute behavioral health needs and develop treatment plans.
- Assess and address behavioral health factors in adult and pediatric patients experiencing acute stress and mood dysregulation.
- Adapt and apply therapeutic strategies to each patient’s unique aspects.
- Provide brief treatment interventions for mild to moderate behavioral health concerns as well as provide crisis intervention.
- Maintain active presence on interdisciplinary care team via collaboration of care with Nurse Case Managers, Pharmacy, Quality.
- Assess patients’ strengths and needs in developing treatment plans and development of coping skills.
- Provide and receive feedback and updates in weekly staff meetings.
- Collaborate with RIPCPC Behavioral Health Referral Coordinators to schedule patients for behavioral health treatment.
- On Call Consultation during the work week to support interdisciplinary departments and participating practices.
- Navigate next steps for patients if continued psychotherapy is needed post RIPCPC short term treatment.
- Provide In Services on relevant clinical topics for behavioral health team.
Clinical Treatment Responsibilities:
- Develop and implement effective treatment protocols for brief solution-focused interventions for adults, children, and adolescents with mental health/substance abuse issues, as well as chronic disease management within a primary care setting.
- Consider and factor necessary cultural competencies through all phases of assessment and treatment.
- Complete initial session and administer standardized clinical tests.
- Develop short term, solution focused treatment plans based on patient’s identified needs and clinical presentation.
- Help coordinate further treatment within the community for ongoing mental health support if appropriate.
- Initial Documentation and subsequent progress notes completed within 72 hours of session in Epic.
QUALIFICATIONS:
Minimum Education and Experience:
- Master’s degree in Social Work or related Mental Health field.
- Licensed in RI (LICSW or LMHC).
- 2-5-years work experience in behavioral health.
- Demonstrated competence in brief assessment, crisis management, triage, and diagnosis, within a brief solution-focused model.
- Experience with Microsoft Office –Outlook, Excel, PowerPoint, Word, Teams.
- Experience with documenting in an electronic health record.
- Excellent oral and written communication skills.
- Highly self-motivated.
- Experience and understanding of co-occurring disorder treatment.
- Ability to work out of RIPCPC office in Cranston, Rhode Island.
Preferred Experience:
- Experience with Electronic Medical records (Epic).
- Bilingual – Spanish preferred.
- Experience working in interdisciplinary teams.
- Knowledge of social and behavioral health resources across the state.
- Clinician must be familiar with virtual- telehealth treatment platforms. Virtual sessions will also be performed in Cranston office.
- Knowledge of ICD 10 coding and DSM V diagnosis.
- Knowledge of PHQ9, GAD 7, C-SSRS completion and interpretation.
