Job Description

The Population Health Care Team Facilitators primary purpose is to execute and manage the population health program work plans, workflows and processes that focus on the population health needs of the underserved patient population.

Essential Duties & Responsibilities

Under the supervision of the Population Health Project Coordinator, the Population Health Care Team Facilitator will:

· Conduct and monitor population health projects

· Design work plans, workflows and processes by which these programs would be managed

· Develop and implement quality improvement initiatives to enhance patient care and outcomes

· Working closely with STPCHC leadership, physicians and care teams to ensure that the population health program goals are successful

· Provides direct services to STPCHC patients, including facilitation, assessment, and case management services, in order to maximize the integration and delivery of population health services across the health centers care departments

· Identify and address any barriers to care delivery and work to resolve any conflicts that may arise with the healthcare team

· Responsible for additional population health management tasks and initiatives defined by management

Education

· Bachelor’s degree in Healthcare, Public Health, Health Promotion, Psychology, Human Development or related field

Experience

· A minimum of two year of experience working as a care coordinator or in providing prevention education, health education, or a similar related service. Working with the underserved patient population is highly preferred.

· Previous experience handling patient interactions or assisting in a healthcare clinic setting

· EMR experience preferred

· Enjoys working in a team-based environment with active collaboration

· Must be an effective communicator with excellent grammar and interpersonal skills

· Bilingual proficiency, English and Spanish speaking

· Strong written communication skills with success in providing notes, updates, and written communications via EMR computer systems

Working Conditions

While performing the duties of this job, the employee is typically required to regularly conduct work in a controlled office environment, moving around the office as needed, sitting at a desk, using office equipment, including but not limited to a personal computer, fax machine, copier and telephone, read, write, listen, speak, and understand English, have the ability to work independently, be dependable and maintain confidential information, integrity and composure at all times.

Constant contact with individuals internal and external to the organization occurs via various methods of communication, typically including face-to-face discussions, electronic mail, phone calls, written letters and/or faxes.

COVID-19 Considerations

· All STPCHC employees must be fully vaccinated against COVID-19 as a condition of employment. Successful candidates for this position will be required to submit proof of vaccination against COVID-19 or request an exemption for qualifying medical or religious reasons during the onboarding process. Candidates should not present proof of vaccination until instructed to do so by the hiring department. All STPCHC employees are required to wear masks in the office irrespective of vaccination status.
