Overview
Join our dedicated team as a Case Manager specializing in Housing Stability, where your passion for social impact and commitment to empowering individuals will drive meaningful change. In this vital role, you will provide comprehensive support to clients experiencing homelessness and/or housing insecurity, guiding them through complex systems and connecting them with essential resources. Your proactive approach will foster stability, promote independence, and help clients overcome barriers to secure and sustainable housing. This paid position offers a dynamic, faith-based environment where your skills in social work, negotiation, and crisis intervention will make a tangible difference in people's lives.

Responsibilities

  • Develop personalized housing plans by assessing client needs, strengths, and barriers using trauma-informed and motivational interviewing techniques
  • Provide crisis intervention and conflict resolution support to clients facing urgent housing or personal issues
  • Maintain accurate case records using HMIS.
  • Conduct regular follow-ups to monitor client stability, adjust plans as needed, and promote long-term housing retention
  • Collaborate with community agencies to connect clients with addiction counseling, mental health services, employment resources, and other supportive programs

Requirements

  • Proven experience in social work, case management, or homeless outreach related field
  • Strong negotiation skills to advocate effectively on behalf of clients and service providers
  • Ability to perform crisis intervention and manage high-pressure situations with professionalism and empathy
  • Experience in case documentation
  • Excellent communication skills including motivational interviewing techniques and crisis management strategies
  • Relevant certifications in addiction counseling or social work are a plus
