The Case Manager is responsible for working with clients to develop a personalized case management plan and continuing case management services. Work involves coordinating programs and activities with other public and private agencies. Responsible professional work is performed under the direct supervision of the Community Services Coordinator.

Key Responsibilities

  • Implements and direct all programs operated by the agency; supervise all employees assigned and subsidized employees of other agencies. Enforces personnel policies, evaluate staff performance, and recommend personnel actions.
  • Interpret programs operated and provide outreach to all community agencies and the general public. Develop and maintain cooperative working relationship with all other public and private social service organizations.
  • Acts a primary source in communicating the needs of low-income citizens to management so that maximum use can be made of funds which are available for programs operated.
  • Prepare reports and other documents as may be required; conduct county wide surveys, maintain records, and recommend changes in program direction, as needed.
  • Work with clients to develop individualized case management plan, which may include home visits and visual inspections.
  • Adheres to strict boundaries and professional ethics.
  • Assist client with program application, act as liaison between client and client service providers.
  • Responsible for all data entry required to capture and track client data and demographics; maintain client logs and fund balances.
  • Attend program-related community coalition and committee meetings and trainings as assigned by supervisor.
  • Possess reliable transportation and availability to travel, including overnight when required.
  • These duties are not all-inclusive and the employee will also perform other related duties as assigned by the department head, supervisor, or other member of management.

Minimum Requirements of the Position

  • Associate Degree in social work or related field required. Bachelor of Social Work or related field preferred.
  • Four years of experience working in a community based organization conducting assessments, utilizing resources in the community to assist low income population preferred.
  • Must be a US citizen; have a Permanent Resident Card, or an alien Registration Receipt Card (Form I-551)
  • Must have and maintain a clean driving record (NOT TO EXCEED 6 PTS)
  • Must have and maintain a criminal history absent of convictions for violent crimes or for any crime against children.

Job Type: Full-time

Pay: $12.50 - $14.00 per hour
