Orion Communities is a non-profit social service organization in Phoenixville that works with the most vulnerable in the community. Orion strives to address unmet needs creatively and compassionately for individuals and families that are experiencing hardship due to poverty, disability or illness by support that leads to self-reliance.

Orion Communities is looking for a professional Case Manager with a positive attitude to assess, plan and implement our neighbors' care.

Job Tittle: Case Manager

Status: Full time - Hybrid

Hours: 5 Weekdays 8 am- 4pm or 9am – 5pm (flexible work schedule)

Reports to: Manager of Case Management

Responsibilities

· Provide short term case management services by performing intakes to determine financial assistance or referrals.

· Present cases weekly for discussion with Case Management Team.

· Develop a plan(s) with client participation and connect with needed community services.

· Establish rapport and develop knowledge of community agencies and county resources.

· Participate in regular staff meetings, training programs and supervision.

· Input accurate and complete data for all contacts with clients into Orion’s database in a timely manner.

· Facilitate/co-facilitate Orion Programming.

· Collaborate and support fundraising and outreach events as requested.

· Collaborate with the Orion team to further Orion’s mission, vision, and values.

· Maintain client confidence and protect operations by keeping information confidential.

Requirements:

  • Bachelor’s degree in social work; related field; or relevant experience with two years minimum.
  • Prior experience in case management or a related field.
  • Experience working with diverse populations, such as individuals with disabilities, mental health issues, or substance abuse problems.
  • Understanding and respecting differences in culture, language, religion, ethnicity, sexual orientation, and socioeconomic status can improve the quality of care and services provided to clients.
  • Exceptional listening skills, to understand what clients need and develop a plan that will work for the client.
  • Exceptional written and oral communication skills.
  • Empathy and the ability to build rapport with clients.
  • Organizational skills, managing all the moving parts of client plans, documenting information, and coordinating services.
  • Problem-solving and critical thinking skills.

· Ability to work independently and seek assistance from supervisor when needed.

· Experience with Microsoft, SharePoint; will be required to learn client database system.

· Bi-lingual (English, Spanish, and Portuguese) a plus.

Salary: $42,000.00 - $46,000.00 per year
