Care Coordinator – Non Medical Home Care
Senior Care At Home is seeking a full-time Care Coordinator to be responsible for overseeing and coordinating care services with clients and care staff and ensuring high quality service. The Care Coordinator must possess a strong customer service orientation, have knowledge of medical conditions and terminology, and be proficient with computer systems and technology.
Responsibilities:
· Along with RN staff, participate in care consultations with prospective clients, gaining an understanding of their needs, address questions and concerns, and build good relationships with clients and family members.
· Coordinate care services: work with staffing team to match/select caregivers, determine schedule with client and caregiver, review care plan with caregiver, and introduce and orient caregivers in client’s home.
· Prepare start of care materials and create electronic and paper employee files.
· Ensure that caregivers are adequately oriented and supervised to deliver client care as outlined in each care plan.
· Discuss financial aspects of Senior Care At Home service with prospective clients, current clients, and stakeholders (family, financial advisors).
· Provide ongoing care management to ensure high quality service delivery and client and caregiver satisfaction. Includes ongoing communication and follow-up activity with clients and employees and managing scheduling changes and updates.
· Oversee the evolution of clients’ needs; update care plans as appropriate, communicate about care plan changes to clients and caregivers, and make home visits as needed for observation, supervision, and customer service purposes.
· Work with RN staff as needed to address changes in conditions, medical concerns, and reassessment needs.
· Document all client service activity and communications in company software system.
· Communicate and reinforce company policies and procedures with caregivers.
· Provide after-hours/on-call support for clients and caregivers. (Some, not all, evenings and weekends)
· Identify caregiver training needs and work with staffing team to make training available to caregivers.
· May represent Senior Care At Home at local industry and community events.
· Maintain positive work environment and employee relationships.
· Participate in employee performance reviews.
· Maintain in office work schedule during agency’s office hours and occasional after hours. Occasional weekend work will be required.
· Represent Senior Care At Home with professionalism, courtesy, and strong customer service focus.
· Report to and regularly convene with owners regarding performance metrics.
· Participate in answering phones and greeting guests in the office.
· Participate in company/team meetings by phone and in person.
· Support a positive work culture and strong team environment.
· Perform administrative/clerical responsibilities relevant to care coordinator job responsibilities.
Job Requirements:
· Healthcare experience required. Experience in patient care coordination preferred; senior (geriatric) care preferred. LPN strongly considered.
· Knowledge of medical conditions and terminology, particularly for older adults
· Available for work on weekends and evenings
· Proficient in the use of computer software, mobile technology and data entry and correspondence.
· Ability to interface effectively and professionally with client stakeholders including family, healthcare providers, attorneys, etc.
· Ability to function in a small team environment as well as autonomously
· Excellent written and verbal communication skills
· Travel in OKC metro area required. Mileage reimbursement is provided for job-related travel.
· Must be comfortable working in homes with pets
· Ability to read, write, speak and understand English
· Possess a valid driver's license and have use of an insured automobile
· Clean criminal background
· Minimum of 2 professional references
