Job Summary:
The Clinical Care Coordinator at Wound Management Specialists plays a key role in managing referrals, facilitating transitions of care, and promoting continuity for patients requiring specialized wound care services. This individual works directly with hospital discharge planners, case managers, physicians, and home health agencies to coordinate timely, appropriate care and ensure patient-centered wound management throughout the treatment journey.
Key Responsibilities:
- Serve as the primary point of contact for hospital discharge planners, case managers, and home health coordinators for referrals to Wound Management Specialists.
- Evaluate patients in the hospital setting for eligibility and appropriateness of in-home wound care services.
- Facilitate timely transitions from hospital to home, ensuring a smooth referral process and clear communication between providers.
- Collaborate with home health agencies to align on care plans and ensure coordinated service delivery.
- Educate patients and families on wound care services, expectations, and follow-up procedures.
- Track and follow up on patients to monitor wound healing progress and ensure care plan adherence.
- Maintain detailed and accurate documentation of all interactions, referrals, and coordination efforts.
- Promote awareness of Wound Management Specialists' services through professional relationships with hospital and agency staff.
Qualifications:
- Active RN or LPN license (state-specific) required.
- Minimum of 2 years of clinical experience in hospital and/or home health settings.
- Prior experience in care coordination, discharge planning, or liaison role strongly preferred.
- Strong knowledge of wound care services and post-acute care pathways.
- Ability to communicate effectively with diverse clinical teams, patients, and families.
- Proficiency with EHR systems and Microsoft Office.

Job Type: Full-time
Pay: $85,000.00 - $95,000.00 per year
