Community Health Worker

Essential Duties and Responsibilities:

• Provide Rural Border Intervention (RBI) program services that allow access to a continuum of substance misuse-related services from evidence-based prevention curricula to post-treatment community based-follow-up for youth and adults at risk for substance misuse.
• Obtain MOU’s with agencies within rural community that serve target population to provide additional support and facilitate referrals for services to program participants.
• Develop activities that are designed to encourage and foster bonding and opportunities for the target population to take part in the educational, cultural, recreational and work-oriented substance-free activities such as community service projects, social outing/events, and health fairs.
• Conduct service planning and coordination, including referrals to appropriate resources and referral follow-up outcomes.

Requirements:

• At least 2 years experience working in social services, networking with social service agencies, or working with at risk populations in the community or schools.
• Traveling required within the service area.
• Current TX driver’s license, reliable transportation, & liability insurance.
• Bilingual (English/Spanish)

Job Type: Full-time

Pay: $31,200.00 per year
