COMMUNITY LIAISON MANAGER

ORGANIZATION SUMMARY:

Western New York WNY Integrated Care Collaborative, (WNY Integrated Care) is an innovative non-profit corporation, incorporated in 2016. As the hub of a multi-county network of community-based organizations (CBOs) and government agencies, we are strategically positioned to respond to and capitalize on opportunities to advance community-based integrated health care and to bring sustainability to community-based programs and services which address social determinants of health. WNY Integrated Care’s role in the network is to provide administrative contracting, billing, technical support, training, and quality assurance on a regional level. The WNY Integrated Care Network works collaboratively to navigate the changing healthcare landscape together to improve health outcomes in our Western New York community.

POSITION SUMMARY: Community Liaison Manager

The Community Liaison Manager is responsible for nurturing and growing the involvement and engagement of community members with regards to all current and future programs and services offered by WNY Integrated Care.

  • Hybrid position: Work from Home / remote office position with in-person team meetings in Western New York/Buffalo.
  • Report directly to Director of Network Development.

ESSENTIAL FUNCTIONS and RESPONSIBILITIES:

  • Assists in the development and implementation of the organization outreach and communication plan for all WNY Integrated Care programs to increase engagement with external stakeholders and community members to ensure programs and services are offered to our diverse population equitably and inclusively.
  • Plans and organizes community outreach events on a regular basis according to a pre-approved plan.
  • Reviews WNY Integrated Care’s referral tracking system with our Operations Team for gaps and opportunities and makes recommendations for improvements.
  • Develops accountability and reporting plans to track and evaluate WNY Integrated Care community outreach and marketing efforts.
  • Manages daily implementation of approved outreach plans and initiatives.
  • Conducts regular outreach to potential referral sources, community members, and community advocates to increase participation in WNY Integrated Care programs.
  • Works collaboratively within the WNY Integrated Care Team environment to ensure the timely completion and reporting of program outcomes and objectives.
  • Trains network members and external stakeholders on use of WNY Integrated Care’s marketing toolkit.
  • Manages multiple time-sensitive priorities.
  • Works independently in a fast-paced environment.
  • Provides support for assigned Committees/ Workgroups of the Board of Directors and the Network.
  • Demonstrates excellent customer service on phone, in-person, via email and in all aspects of employment at WNY Integrated Care
  • Communicates in a positive manner both internally and externally to partners.
  • Excellent oral and written communication skills.
  • Represents WNY Integrated Care at local, state, and national conferences, meetings and events.

QUALIFICATIONS:

  • Minimum of an associate’s degree preferred in Marketing, Public Relations, Management, Business Administration, or related field; relevant life experience may be considered in lieu of degree.
  • Minimum of three years of successful experience working in a variety of community, public health, non-profit, or similar operations.
  • Experience giving dynamic public speaking presentations and leading trainings.
  • Strong computer/technology skills including: Microsoft Office-Suite, Outlook, surveys, and a Customer Relationship Management (CRM) platform (such as HubSpot).
  • Must be organized, able to work on multiple projects, and willing to take on new projects or tasks throughout WNY Integrated Care.
  • Ability to relate effectively to diverse groups of people from all social and economic segments of the community with a track record of building authentic, constructive relationships with others.
  • Strong analytical and critical-thinking skills with the ability to identify and leverage relevant data to use in planning and decision-making.
  • Team player who enjoys working both collaboratively and independently.
  • Must have a reliable form of transportation, as a significant portion of the outreach will be conducted in the field throughout the Western and Central New York regions.
  • Excellent oral and written communication skills.
  • Able to professionally represent WNY Integrated Care at local, state, and national meetings, conferences, and events.
