SUMMARY: Provides individualized care and services that is “people centered” with the goal that clients/participants have the best day possible in a safe and caring environment. In addition – the Day Service Assistant will assist with a variety of age and interest appropriate social, emotional, cognitive, and physically stimulating programs such as crafts, music, movies, games, exercise, holiday events, outings etc. based on the needs and goals of our clients. May be trained for Job Coaching in the community as needed and will include driving bus, van or car to ensure safe, reliable transportation on a scheduled basis in order for clients to participate in programing.

Essential Functions, Duties and Responsibilities: Duties may be modified or additional duties assigned

· Works with Day Service Coordinator to ensure clients participate in daily/weekly/monthly schedule of programs, outings, events and activities based on needs, abilities and interest to clients; develops, implements, and evaluates programing that impact client lifestyle, promotes positive behaviors, stimulation and meaningful socialization on a 1:1 basis or in groups. Alerts to equipment or supplies needed.

· Maintains all safety protocol and ensures client area is clean and sanitized per policy and procedure.

· Provide a safe care and transportationfor participants at all times by such practices as; Proper operation of wheel chairs and all other medical/ mobile devices needed.

· Assists with personal careas needed; to maintain an optimal level of functioning, cleanliness, eating/meal preparation, toileting, ambulation etc. Ensure all medications are given properly according to the medication protocol, ensure safety during lunch and snack breaks.

· Application and maintenance of orthotics as prescribed.

· Redirection, monitoring, observation reporting is an integral part of a personal care/behavioral care plan for clients; Reports frequency and severity to Director of Client Services and/or Case Manager.

· Works with volunteers when scheduled for activities and events.

· Researches out new ideas for activity development; will work with co-workers to implement meaningful programs.

Pay: $16.15 - $17.27 per hour
