Full job description

Job Title: Family Navigator Case Manager

To support our mission, Hotel Hope is seeking a compassionate individual to join our team who respects the dignity of all human beings and believes in providing opportunity to all. You will be responsible for aftercare case management tasks, including building trusting relationships with the families you serve. The ideal candidate will have proven experience in working with families who have experienced homelessness in the GNO area. They will be driven to serve the less fortunate and be a standard bearer of Hotel Hope’s values. This individual will work closely with both the Case Manager and the Director of Operations.

Reports to: Director of Program Services Classification: Exempt / Full Time Salary

Job Summary: Under the direction of the Director of Program Services, the family navigator provides intensive case manager services and appropriate supportive housing for family’s transition from the emergency shelter to permanent housing. After the family is referred by UNITY of Greater New Orleans for rapid rehousing, the family navigator will formulate case plans to promote a plan toward self-sufficiency. The family navigator will work on an intensive case management plan when family is transiting to the Aftercare Program and will work with the family up to 9 months.

The Family Navigator uses the Critical Time Intervention (CTI) model to assist family members in the transition. Using the CTI model the family navigator assists with budgeting, home organization, parental support and provides additional resources as needed. Additionally, the Family Navigator Case Manager will make regular contact to family with home visits, phone calls and emails to assist the family in returning to family routines.

Duties and Responsibilities:

Family Navigator Case Manager

· Directly supports Hotel Hope’s mission, vision, values and goals

· Follows protocol and guidelines set by the Program Services office

· Understands and implements the Critical Time Intervention model for case management services.

· Assists in setting and monitoring the implementation of direct services for each family in setting and coordination of case management plans in cooperation of the Director of Program Services.

· Coordinates services for each family case plan beginning with the assignment of the outside agency’s Rapid Rehousing Case manager.

· Builds a relationship with family at time of Rapid Rehousing referral and continues to work with family for 9 months after emergency housing stay.

· Makes appropriate referrals

Case Plan:

· Develops a comprehensive guest-driven case plan with short-term and long-term goals identified for 9 months after leaving Hotel Hope MLK

· Completes a timeline for each family and outcomes for each goal

· Monitors progress toward goals with regularly visits and contact with guests.

· Evaluates and adjust case plans as needed and provides appropriate referrals

· Refers guests to appropriate resources to assist with goals

· Continues on-going intensive case management with guests who move to permanent housing

Recordkeeping and Recording

· Maintains client files to include conversations, progress toward goals, documentation of any incidents and warnings

· Report critical incidents immediately to Executive Director

· Collects data necessary to meeting funding requirements and statistical reports for all grants

· Completes daily case management log and termination submits weekly report to supervisor

Teamwork and Collaboration:

· Works in collaboration with all staff to facilitate a team environment

· Participates in Unity Homeless Services meetings and commits to group decisions

· Models effective team behaviors

· Demonstrates effective communication skills in building relationships with all employees and guests and other nonprofit service providers

· Creates good working relationships with other service providers to facilitate the access to local resources for guests

· Substitutes for another staff when need arises
