Full job description
Program Leadership and Supervision
- Implement, reinforce, and continually reassess the Grace House program model of community accountability and individual support for community residents.
- Maintain program schedule of internal and external supports for Grace House residents, including both individual and group services.
- Supervise and support key case management duties including intake, individualized case plans, structured case management sessions, documentation of participant progress, and monitoring compliance with program and parole requirements.
- Work with Aftercare and Alumni Coordinator to support effective resident transition planning and aftercare alumni supports (One St. Leonard’s for Life).
- Ensures SLM Diversity, Equity, Inclusion, and Belonging principles are visible and alive in the Grace House Community.
- Work with Manager of Intake and Housing to maintain resident census, manage referral processes and communications, and ensure appropriateness of resident fit with Grace House model and services.
- Be a visible, supportive, and consistent presence within the Grace House and overall St. Leonard’s Ministries communities.
Employee Supervision, Development, and Support
- Supervise all Grace House employees and work closely with them to build their skills and confidence.
- Leads a culture and models behavior that contributes to teamwork, support, professionalism, and responsiveness.
- Conduct individual employee evaluations, individual development plans, and enact progressive discipline as necessary.
- Manage all aspects of staff scheduling to ensure 24-hour coverage.
- Support staff and residents in crises as needed.
Integrated Care and Program Supports
- Implement and be accountable to St. Leonard’s Integrated Care Framework by coordinating and communicating with key partners.
- Continually assess integrated care partners, services, and community member needs, and make recommendations for additions, changes, or removals as needed.
Fiscal and Operational Impact
- Participate in annual and grant budget development processes and maintain a high level of fiscal accountability.
- Provide data and participant information for the purpose of compliance, program evaluation and grant reporting.
- Work with Director of Quality Improvement and Grants to develop and implement program evaluation protocols: set program goals and objectives, implement and track data, and evaluate results against these goals.
- Ensure that the expectations of funders, partners, constituents, participants, and other stakeholders are consistently met.
- Develop and implement appropriate individual and programmatic risk management plans and monitoring.
- Coordinates and communicates effectively and respectfully with other SLM departments (facilities, kitchen, HR, etc.).
